The maximum out-of-pocket expense is not a deductible, it is the accumulated Limit that you can pay once a year for the sum of all types of operations, surgeries or other types of expenses incurred during the time covered by your policy.
In Obamacare plans there are two types of plans: EPO (you can go to your specialist doctor without going through a primary doctor) and HMO (in order to consult a specialist doctor, the primary doctor has to authorize you).
